The Footprint of a Nursing Home Leader
Many of us promised our parents that we wouldn't ever put them in a nursing home. Reality is that more than 3 million Americans rely on services provided by nursing homes at some point during a year and 1.4 million of these reside in the Nation’s 15,800 nursing homes on any given day (Bonner, 2013). In my position as Vice President of Operations with a publically traded healthcare company, I support twelve nursing homes. One such nursing home is a 194-bed for-profit Center that provides post-acute, rehabilitative, skilled nursing, and short and long-term care services. This Center is located in Texas approximately 5 miles from the border of Mexico. In an effort to support the Centers’ mission to “Make a Difference – Every Day, Every Time,” there are six key objectives vital to success: 1. Culture: Maintain a productive and positive employee environment. 2. Care: Provide quality care so residents may achieve their optimum level of functioning thus living an improved quality of life. 3. Census: Anticipate community health needs and develop business partnerships that help improve market share. 4. Cash: Ensure timely and effective collections. 5. Cost: Control operation costs. 6. Compliance: Comply with regulatory requirements.
Strong leadership is essential for this Center to successfully reach its key objectives. While the leadership at this Center has a history of consistently attaining key objectives, over the course of the previous two years this Center has experienced increasingly greater difficulties at doing so. The goal of this paper is to provide an analysis of this Center’s leadership to include the organization strengths, weaknesses, opportunities, and threats.

...Nursing Home Abuse Nursing homes are meant to be a safe haven for the elderly to cared for in their golden years and instead of become a nightmare for a lot who have been subjected to countless acts of abuse and neglect. Furthermore, the significant others of those in nursing homes are subjected to highly stressful conditions by having to meet harsh economic requirements. According to the National Center on Elder Abuse, there are nearly 17,000 nursing homes in the United States that currently care for 1.6 million residents, a figure expected to quadruple to 6.6 million residents by 2050. In past years, there have been reports that nearly 1/3 of all nursing homes have been cited for abuse violations. Each year the number of incidents increases while there is a significant underreporting of nursing home abuse and neglect. More than twice as many nursing homes were cited for abuse in 2006 and with numbers expected to quadruple these numbers are going to rise. Reports show that there are thousands upon thousands of abuse cases yearly and investigators believe many violations go undetected or are simply not reported, leading federal regulations to believe the problem is underestimated. Even when serious issues have been identified federal and state enforcement policies have not been effective in ensuring that the issues are corrected and remain corrected. ...Nursing Home or Not? When thinking about moving a parent into the home, it must be thought thoroughly. This feeling may be because one doesn't want their parents in a nursing home, because they want their kids to bond with their grandparents, or because it is the only financially reasonable option that can be thought of. It could be out of love and compassion, or a sense of responsibility or guilt. Whatever someone’s motivation, making that move will have an intense impact on your life. This may be a great opportunity for the children to form close ties with their grandparent. The United States today is such a mobile society that children often don't get much chance to be around the older members of their family. While I was visiting my aunt one day, I witnessed one of the funniest incidents I had ever seen. As I approached the nursing station, I saw the elevators doors open. This is the elevator that comes up from the bottom floor (Magnolia Terrace) which houses the Alzheimer’s patients. Out of the elevator came an elderly lady dressed like you wouldn’t believe. She had on a striped blouse, a skirt covered in big flowers, yellow socks, and the most god awful plaid tennis shoes. Her makeup appeared ridiculously messy, and her hair reeked of terrible disarray. The nurse jumped up from her seat, and hurried to the elderly lady; she said, “Oh honey, you need to go back down to your floor.” The elderly lady kept refusing to get back on the elevator and go back down. After approximately three times of the nurse urging the elderly lady to go back down the elevator; the elderly lady shouted, “I need to get to my car, go home, and rest. I have been downstairs visiting my sister for the last two hours, and I don’t have time for this nonsense.” I will never forget the look on that nurse’s face as long as I live. ...were abused or neglected (Wallace, 2005). While the majority of abuse happens in the home by a family member, there is a rise in the amount of elderly abuse being reported in nursing homes. In 2000, states were asked to indicate the number of elder abuse reports received from nursing homes. Based on figures from 54 states, the total number of reports was 472,813 (Teaster, 2003). A study prepared by the minority staff of the Special Investigations Division of the House Government Reform Committee found that 30% of nursing homes in the United States, 5,283 facilities, were cited for almost 9,000 instances of abuse from 1999 to 2001 (Waxman, 2001). The report stated that 1,601 of these instances could have or did cause the resident serious injury or death. The instances reported ranged from physical abuse, verbal abuse, sexual abuse, bedsores, inadequate medical care, malnutrition, dehydration, preventable accidents, inadequate sanitation and hygiene. Some other shocking finds of the report were that more than 40% of the abuse violations were not reported until formal complaints was brought by residents or family members. 1,327 of the nursing homes were cited more than once for abuse violations during this two year period, 305 were cited for three or more violations and 192 were cited for five or more abuse violations (Waxman, 2001).
